Initial Intake Evaluation (60 minutes): $275
60 minute session: $200
45 minute session: $175
Sliding Scale
A sliding scale will be offered to self-pay patients based on house hold income if needed. If you do not think that you can afford my full fee please feel free to reach out to me directly to discuss payment options.
Payments
I accept cash, checks, credit cards and HSA payments.
All Payments are due at the time of service.
Insurance
In-Network: United Healthcare, UBH, UMR, Cigna, Lakeland, Forward Health, Health Partners and Anthem Blue Cross and Blue Shield starting 12/1/2020
Insurance rates may vary based on individual plans and coverage. Please check with your insurance company prior to starting therapy. Please note; just because I am not in-network with your insurance plan does not mean that services will not be covered. In this case, you may pay a higher rate or deductible. Co-pays are due at time of service.
Cancellation Policy
Please notify me 24 hours in advance if you cannot make it to your scheduled session. Clients that do not notify me 24 hours in advance and do not make it to their session could be charged in full.
